One important test is the SpermComet test, which looks at the quality of sperm DNA. Unlike the standard sperm analysis, which evaluates count, motility, and shape, the SpermComet test provides insight into DNA integrity, a key factor for successful fertilisation and embryo health. When combined, these tests provide a full picture of male reproductive health.

The good news is that sperm regenerate approximately every 90 days. Many couples who have been told they need ICSI (Intracytoplasmic Sperm Injection) for fertilisation find that after making suggested lifestyle changes, they are able to conceive naturally or with less invasive treatments.

A report of low AMH (Anti-Müllerian Hormone) is often a source of great anxiety for many people on their fertility journey. This test is commonly included as part of a ‘Fertility Assessment’, but it is poorly understood and often leads to unnecessary stress. If you’ve just discovered you have low AMH, let us take a step back and understand what this really means and what actions you can take.

First, let’s start with five key points to keep in mind:

  1. AMH says very little about your natural fertility or your chance of conceiving naturally.

  2. AMH is not an indicator of egg quality — egg quality is mostly influenced by age, rather than AMH levels.

  3. AMH may help IVF clinics predict the number of follicles available for collection during treatment.

  4. AMH can guide an IVF clinic in deciding the right stimulation protocol for you, which influences how your ovaries are stimulated.

  5. AMH levels differ between races, and this has no impact of differences in egg numbers, quality or conception rates.

I’ve Been Told I Have Low AMH, What Now?

If you’ve just found out that your AMH levels are low, don’t panic! Low AMH does not mean your journey towards starting or growing your family has come to an end. It’s simply a sign that we need to investigate your fertility more closely. Low AMH can indicate that there may be underlying issues affecting your ovarian environment or overall reproductive health, and addressing these issues can make a big difference.

There are several potential factors that may impact the health of your ovarian environment, including:

  • Vitamin D Deficiency: Low levels of vitamin D have been linked to reduced ovarian function.

  • Anaemia and Low Iron (Ferritin) Levels: Low iron can impact overall reproductive health and vitality.

  • Inflammation: Conditions such as IBS, IBD, Crohn’s, or arthritis can contribute to a suboptimal ovarian environment.

  • Microbial Imbalances: Infections or dysbiosis in your gut, vaginal, or uterine microbiome can cause inflammation that impacts fertility.

  • Poor Circulation: Issues like pelvic tension, surgical scars, adhesions, or postural problems can reduce blood flow to the reproductive organs.

  • Autoimmune Conditions: Conditions such as Hashimoto’s or lupus can negatively affect reproductive health.

  • Certain Medications: Some medications, including the contraceptive pill, can impact ovarian function.

  • Smoking and Poor Sleep: Both can have adverse effects on your ovarian health.

  • FSH, LH, and Oestrogen: These hormones should be tested on day two or three of your cycle to gain insight into ovarian function.

  • DHEA-S and Testosterone: Low levels of these hormones can impact ovarian output, potentially leading to lower AMH.

2. Properly Assess Your Thyroid Function

Many standard thyroid tests do not provide the full picture. It’s important to also test T3 and thyroid antibodies. Elevated thyroid antibodies can indicate autoimmune activity, which may increase the risk of premature ovarian insufficiency. This is particularly crucial if there is a family history of early menopause or autoimmune conditions.

3. Get an Ultrasound from a Specialist

An ultrasound with a specialist in women’s gynaecological health can help determine if your ovaries look healthy in terms of size, shape, and activity. An ultrasound can also rule out other fertility issues, like cysts or fibroids, that could be impacting ovarian health.

4. Investigate Male Fertility Thoroughly

If your AMH is low, it’s important to also look at your partner’s fertility. Many people with low AMH are still highly fertile, so ensuring there are no male factor issues is critical. A DNA fragmentation test can be useful as part of this assessment.

How Does Traditional Chinese Medicine View Low AMH?

In Traditional Chinese Medicine (TCM), low AMH is often related to Kidney Yin Deficiency. Yin represents the cooling, nourishing energy of the body, and is essential for optimal reproductive health. Kidney Yin deficiency can manifest in symptoms such as restlessness, frequent waking at night, hot palms and feet, or a cracked, red tongue.

In TCM, Kidney Yin is associated with oestrogen, FSH, and AMH levels. If Yin is deficient, AMH levels may be low, FSH may be high, and oestrogen could also be affected. This impacts the follicular phase of the menstrual cycle, as well as egg quality and the uterine lining. TCM treatment aims to replenish Kidney Yin, support overall hormone balance, and create a nurturing environment for follicle development.

Pelvic Congestion Syndrome (PCS)

Such symptoms may indicate Pelvic Congestion Syndrome (PCS), also known as Pelvic Vein Incompetence and is commonly associated with varicose veins located in the pelvis.

Research suggests PCS affects about 30% of women investigated for chronic pelvic pain, although from my colleagues and my experiences, it seems less common that this, at least with women seeking natural help.

 

What is Pelvic Congestion Syndrome?

Pelvic Congestion Syndrome is a chronic ailment caused by dilated veins within the pelvis, akin to varicose veins that are typically found in the legs. This condition primarily manifests as a deep, aching pain that worsens throughout the day, particularly after prolonged standing, although it can be asymptomatic. The pain stems from blood pooling around the ovaries and the vulvovagina caused by venous obstruction, problems with valves in the blood vessels, and/or changes in hormone levels, all leading to increased pressure and discomfort in the pelvic area.

Despite being a prevalent cause of chronic pelvic pain, PCS frequently goes undiagnosed due to its symptoms mimicking those of other medical conditions like endometriosis, uterine fibroids (leiomyoma), interstitial cystitis, lower back/hip/leg pain and ovarian cysts. It is essential to note that PCS is not exclusive to women; men can also be afflicted with similar chronic pelvic pain due to this condition. The male equivalent of this syndrome is called Variocele and is the vein incompetence in the testes, this seems to be more commonly known of and treated more frequently within the fertility field.

Pelvic pain related to vascular issues such as PCS can be complex and is often not well-understood in conventional medical practices. This leads to general treatments that may not effectively target the specific problems causing the pain.

Diagnostic techniques such as Doppler ultrasound or venography are often employed to pinpoint the root causes of venous problems, which can be addressed through hormonal treatments, vein embolization, or surgical procedures.

Medications used to suppress estrogen, which can alleviate symptoms, might include Depo-Provera, Nexplanon (a common alternative to Implanon in the UK), or Zoladex (commonly used instead of Goserelin). These medications also act as contraceptives, hence they may not be suitable for individuals who are trying to conceive or prefer to avoid hormonal contraceptives for personal reasons.

Ovarian vein embolisation (OVE) is a surgical technique that obstructs veins to prevent the accumulation and stagnation of blood in the pelvic region. Studies indicate that OVE does not significantly affect ovarian function, hormone levels, or the ability to conceive. In fact, many women have successfully become pregnant after undergoing the procedure.

Some research has demonstrated that about two-thirds of women who underwent OVE for pelvic congestion syndrome experienced successful pregnancies following the treatment. Additionally, hormone levels such as follicle-stimulating hormone, estradiol, and luteinizing hormone have been shown to remain stable post-procedure, suggesting that ovarian functionality is preserved

Your PCS symptoms will be as unique as you are, however these are some of the most common symptoms to look out for:

  • Progressive Pelvic Pain: Throughout the day, individuals often experience an intensifying pelvic pain due to blood accumulation in the pelvic veins, which increases pressure.
  • Pain from Prolonged Standing: Extended periods of standing tend to exacerbate the sensation of pelvic pressure and pain.
  • Postprandial Pain: Consuming meals might provoke pelvic and abdominal pain, alongside bloating, contributing to discomfort during and after eating.
  • Urinary Incontinence: Challenges in controlling urination can occur, especially during activities like coughing or lifting heavy objects.
  • Painful Urination: Urination may become painful or produce a burning sensation, sometimes mistaken for a urinary tract infection (UTI).
  • Urinary Urgency: There may be a sudden and intense need to urinate, mimicking symptoms of other bladder issues.
  • IBS-Like Symptoms: Fluctuating between constipation and diarrhea, coupled with bloating and abdominal pain, reflects symptoms similar to Irritable Bowel Syndrome (IBS).
  • Lower Back Pain: Pain may radiate in the lower back area, often confused with conditions like sciatica or general musculoskeletal issues.
  • Leg Discomfort: Standing for long periods can lead to throbbing, aching, and swelling in the legs, indicative of poor circulation.
  • Pain During Intercourse: Increased pelvic vein pressure can cause pain during or after sexual intercourse, a condition known as dyspareunia.
 

How Can Acupuncture Support Pelvic Obstruction Syndrome?

Traditional Chinese Medicine (TCM), and specifically acupuncture, is recognised as an effective treatment for conditions such as PCS and Chronic Pelvic Pain Syndrome (CPPS).

Acupuncture is embraced globally for its efficacy in managing pain syndromes. The primary mechanism of acupuncture lies in its stimulation of specific acupoints, enhancing local blood flow and decreasing inflammation, thereby significantly reducing pain.

The muscles and fascia within the pelvis are vital for maintaining pelvic and spinal stability. These structures can be adversely affected by trauma, postural stress, pregnancy, anxiety, and other subtle influences.

These complexities are why pelvic pain and congestion often pose a challenge for conventional medicine. Visceral pain within the pelvis may not be immediately apparent, as it can arise from microtraumas, adhesions, scar tissue, and referred pain from various locations.

Acupuncture treatment can mitigate this type of pain, with the insertion of needles at specific acupoints providing both analgesic and soothing effects.

Research has shown that acupuncture releases neurochemicals such as adenosine, which have an analgesic impact during acupoint stimulation, thus alleviating pain in conditions like CPPS. One particular study observed male patients with pelvic pain and venous congestion. Following five weekly sessions of acupuncture, there was a marked reduction in pain, with MR venography confirming a decrease in intrapelvic venous congestion.

Tailored formulations of Chinese herbs are also crafted to target the underlying causes of pelvic congestion, providing relief from the symptoms.

Ovarian Hyperstimulation Syndrome (OHSS)

Ovarian Hyperstimulation Syndrome (OHSS) can be a significant risk associated with in vitro fertilisation (IVF), particularly among those using hCG (Human chorionic gonadotropin) medication during assisted reproductive treatments (ART). Affecting around 3-5% of women undergoing these procedures, OHSS is characterised by an exaggerated response of the ovaries, potentially leading to serious health complications.

 

Understanding the Ovarian Response

During OHSS, the ovaries become enlarged and fill with fluid, releasing excessive amounts of hormones like oestrogen and progesterone, which increase vascular permeability—allowing fluids to leak into the abdominal cavity. In severe cases, this leakage can extend into the lungs and heart.
 

OHSS and PCOS

Women with Polycystic Ovary Syndrome (PCOS) are particularly susceptible to OHSS due to the unpredictable ovarian response to fertility medications.
 

Impact on IVF Cycles

OHSS can disrupt the course of an IVF cycle, potentially halting embryo transfers and necessitating the freezing of eggs for future use. While mild cases can be managed and may not require cycle discontinuation, moderate to severe instances necessitate immediate medical intervention and possible hospitalisation.
 

Stages of OHSS and Management Strategies:

Mild OHSS: Symptoms include abdominal bloating, nausea, slight weight gain, and diarrhoea. What should I do? 
  – Inform your fertility clinic 
  – Treat with acupuncture 
  – Treat with Chinese herbal medicine 
  – Avoid intense exercise and sex 
  – Light activity only 
  – Rest with legs raised 
  – Drink at least 10-12 glasses of fluid per day, especially drinks that contain electrolytes, such as celtic salts 
  – Avoid alcohol or caffeine 
  – Take an over-the-counter pain relief 
 
Moderate OHSS: Symptoms escalate to more significant bloating (which would increase the abdominal measurement), rapid weight gain (more than 2lbs per day), vomiting and diarrhoea, scanty and dark urine, excessive thirst.

What should I do?
  – Seek medical evaluation
  – You may be prescribed Cabergoline
  – Continue to follow the advice above if you are sent home
  – Inform your fertility clinic
 
Severe OHSS: Symptoms become critical. Fullness and bloating above the belly button, shortness of breath (fluid collecting in the lungs), urine reduction or darkening, calf and chest pains, lower abdominal pain, stools which are difficult to pass.

What should I do?
  – Call an ambulance immediately
  – Inform your fertility clinic
  

Role of Chinese Medicine in Managing OHSS

Traditional Chinese Medicine (TCM) offers a unique perspective on OHSS, attributing it to patterns of Damp or Damp-Heat, often stemming from a Qi deficiency. This deficiency hampers the body’s ability to manage the increased ‘heat’ from ovarian stimulation drugs, leading to fluid stagnation or ‘Damp’, and in some cases, a combination of Damp with Heat, manifesting as Damp-Heat.
 

Preventive and Supportive Care Through TCM

Acupuncture: This ancient practice is known for its effectiveness in regulating blood flow and managing pain. By stimulating specific acupoints, acupuncture can help mitigate the risk of OHSS by improving blood circulation and reducing inflammation. Herbal Medicine: Unlike one-size-fits-all solutions, Chinese herbal medicine provides personalised treatments tailored to address individual imbalances. For those at risk of OHSS, formulations typically include ‘damp-resolving’ herbs to move the stagnating fluids.
 

Who Is At Risk?

  – Are young with naturally high hormone levels.
  – Respond quickly to stimulating drugs and have high oestrogen (E2)
  – Are donating eggs (medication likely to be increased)
  – Have a TCM diagnosis of Damp, Spleen Qi Deficiency and possibly also Kidney Yin Deficiency. (Kidney Yin Deficiency means there is already increased heat in the body. This is exacerbated with the stimulation medication.)
  – Have PCOS – During IVF stimulation phase, FSH and hCG are injected at high doses, creating a risk of superovulation. Women with PCOS have multiple half-ripened or unripened follicles which suddenly all develop.

Steps to Take

Pre-IVF Preparation: Engaging with a TCM practitioner to assess and possibly treat any underlying Damp or Qi deficiency can significantly reduce the risks associated with OHSS. During IVF: Incorporating weekly acupuncture can help manage the side effects of medications and support the body’s response to hormonal treatments. The Chinese herbal formulas prescribed will be safe to use alongside IVF medications and even in pregnancy, when prescribed by a licensed herbalist. If you suspect that you have high hormone levels, get a blood test or ask your fertility doctor the results of your day 1-3 blood test.

Every menstrual cycle is unique, just like every person. What’s normal for one individual may not be for another. However, there are general parameters that a healthy cycle should fit within to be considered optimal for fertility:

  • Menstrual Cycle Length: The length of your cycle (from the first day of your period to the first day of the next) can range from 24 to 36 days. The average is 28 days, but this isn’t a one-size-fits-all number. Variations from month to month are also common.
  • Follicular Phase: This phase starts on the first day of your period and lasts until ovulation. During this time, a follicle matures an egg ready for release. It typically lasts between 10 and 20 days.
  • Ovulation: Ovulation occurs once the follicle is ready and the egg is mature. It can happen anywhere from day 10 to day 20—don’t assume it always occurs on day 14. There should be noticeable cervical mucus for 2 to 7 days before ovulation. This mucus is your best indicator of fertility.
