This Privacy Policy explains how I collect, use, and store your personal data in compliance with the General Data Protection Regulation (GDPR) 2016/679.
Why I Collect Your Data
I collect personal information to provide you with the best possible treatment. This includes:
- Health Information: Necessary for effective and safe acupuncture treatments/herbal prescriptions.
- Contact Information: To confirm appointments and update you on your medical care.
- Health Updates: With your consent, I may send individualised health information by email. You can withdraw consent at any time.
Legal Basis for Processing Your Data
- Contract: Your request for treatment and my agreement to provide care.
- Legitimate Interest: Ensuring effective practice and communication.
- Consent: For sending health information (verbal consent is sufficient).
Data Retention
I retain your records for 8 years after your last appointment or until you turn 25, whichever is longer. After this period, your data will be deleted.
Data Security
Your clinical records are securely encrypted and stored on Cliniko, a GDPR-compliant software. Emails are stored on my password-protected computer. I am the sole person with access to your records and emails.
Sharing Your Data
Your data will not be shared without your written consent, except where legally required. If you are referred by another practitioner, I will seek your permission to update them.
Your Rights
- Access: You have the right to see the personal data I hold about you.
- Correction: You can ask to correct any factual errors.
- Response Time: I will respond to data requests within 30 days, but usually much sooner.
Complaints
If you believe your data is being mishandled, please contact me first. You can also raise concerns with the Information Commissioner’s Office at ICO Concerns.
Additional Policies
- Refusal of Treatment: I reserve the right to refuse treatment in cases of aggressive or inappropriate behaviour.
- CCTV: CCTV operates outside the clinic’s internal door. Footage is stored for 30 days by the building’s owner and may be shared with authorities if necessary.
- Cancellation Policy: 48 hours’ notice is required for cancellations. Cancellations within 48 hours will incur the full fee. Being late by more than 15 minutes will be considered a ‘no-show’ and charged accordingly. If late, your appointment will end at the original end time.
- Communication: Use the clinic mobile for calls and messages within clinic hours. For out-of-hours contact, email LauraTheAcupuncturist@gmail.com.
